DHA makes up more than 40% of the total omega-3 fatty acids in neuronal tissue, concentrated especially in the gray matter — it’s not a minor structural component, it’s a defining one. A 2022 systematic review in Cureus pooled 9 randomized controlled trials and 1,319 participants specifically to test what omega-3 supplementation, in its DHA and EPA forms, actually does for brain function.

Why DHA and EPA Specifically

Of the three main omega-3 types, DHA (docosahexaenoic acid) and EPA (eicosapentaenoic acid) are the ones that matter most for the brain — DHA alone accounts for roughly 40% of total omega-3 PUFAs in neuronal tissue, and 50–60% of the brain’s weight is lipid, a substantial share of it omega-3. DHA accumulates rapidly in the brain during late pregnancy and the first 18 months of life, which is why it’s routinely added to infant formula. In adults, omega-3s are thought to support cognition partly by changing membrane fluidity to increase neurotransmitter release, and partly by reducing neuronal apoptosis (programmed cell death) through decreased reactive oxygen species and downregulated apoptotic protein expression.

What the Nine Trials Found

Memory: The Clearest Signal

In one 24-week trial, 900 mg/day of DHA significantly reduced memory errors on a standardized learning task compared to placebo, with serum DHA concentrations quadrupling and correlating with better memory scores — a well-tolerated intervention with no major side effects reported. Multiple other trials in the review found similar memory benefits, particularly in older adults (ages 50–75) and in people who were lonely, a population shown to be more vulnerable to episodic memory decline.

Brain Blood Flow and Executive Function

Several trials measured oxyhemoglobin and blood flow in the brain directly. A 2g/day fish oil dose significantly increased oxyhemoglobin and hemoglobin activity during cognitive tasks compared to placebo, and a 26-week, 2.2g/day trial found omega-3 improved executive function by 26% compared to no change in the placebo group.

Where the Effect Was Weaker or Absent

Not every trial found a benefit. An 18-week trial using a moderate 300 mg/day dose found no overall effect on cognition, though it did improve executive function specifically in participants with low baseline DHA levels. A 6-month trial in cognitively healthy adults found no change across four cognitive domains overall, though participants who started with low episodic memory scores did improve significantly. The pattern across trials suggests omega-3 helps most in people who are starting from a lower baseline — whether in DHA status or cognitive performance — rather than boosting an already-adequate brain.

Dose and Duration Matter, Not Just “Taking Omega-3”

The review notes that dose and treatment duration are essential variables: one trial found that 26 weeks of EPA and DHA had no impact on cognition at all, while smaller doses over shorter periods have sometimes shown less transport of omega-3 into the brain. Adherence and individual metabolism also varied — plasma phospholipid levels reached saturation after only five weeks of 2g/day DHA in one referenced study, suggesting there may be a practical ceiling to how much extra DHA gets used by the brain at a given dose.

The Bottom Line

Across nine controlled trials and over 1,300 participants, omega-3 supplementation showed a real, if not universal, benefit for memory, cognitive well-being, and brain blood flow — the review’s own conclusion states omega-3 therapies are “beneficial, well-tolerated, and very low-risk.” The benefit was most consistent in older adults, people with low baseline DHA or cognitive performance, and lonelier individuals — populations plausibly starting with more room to improve — while some trials in healthy, higher-baseline populations showed a smaller or null effect on cognition overall.
