Biotin for Hair and Nails: What the Evidence Actually Shows

Biotin (vitamin B7) is one of the most heavily marketed hair-and-nail supplement ingredients on the market, largely because it’s inexpensive and biologically plausible — it’s a required cofactor in protein synthesis, including keratin production, the structural protein hair and nails are built from. A 2017 review in Skin Appendage Disorders did something the marketing rarely does: it systematically checked what the published clinical literature actually supports.

What the Review Found

Searching the published literature, the authors identified 18 reported cases of biotin supplementation improving hair or nail growth. Every single one of those 18 cases involved a patient with an identifiable underlying cause of poor hair or nail growth — most commonly an inherited enzyme deficiency (biotinidase or holocarboxylase synthetase deficiency), with a smaller number of cases involving uncombable hair syndrome or brittle nail syndrome. In every one of these cases, biotin supplementation was followed by measurable clinical improvement.

The Part That Doesn’t Get Repeated as Often

The review’s central finding is what it didn’t find: there have been no randomized controlled trials demonstrating that biotin supplementation improves hair or nail growth in healthy individuals without an underlying deficiency or pathology. In vitro research cited in the review found that biotin does not influence the proliferation or differentiation of normal, non-pathologic hair follicle cells — meaning there isn’t even a clean mechanistic basis for a healthy-hair-follicle effect, as opposed to a deficiency-correction effect.

How Common Is Actual Biotin Deficiency?

Genuinely rare. True biotin deficiency in healthy adults eating a balanced diet essentially isn’t documented in the literature — typical Western dietary intake (35–70 mcg/day) comfortably covers the adequate intake level of 30 mcg/day for adults, and biotin is also produced by normal gut bacteria. The recognized causes of deficiency are specific: inherited enzyme deficiencies, certain anticonvulsant medications (like valproic acid), chronic raw egg white consumption (the protein avidin binds biotin and blocks its absorption — cooking denatures it), and prolonged antibiotic use disrupting gut flora. One study measuring biotin levels in 541 women complaining of hair loss found 38% had low biotin — but 11% of those had an identifiable cause (medication, GI disease) and 35% had a co-existing skin condition, suggesting the low biotin often wasn’t the actual cause of the hair loss.

The Bottom Line

The evidence genuinely supports biotin supplementation for one specific situation: a confirmed or strongly suspected biotin deficiency, whether inherited or acquired. Outside of that — for someone with a typical diet and no underlying deficiency — the review’s own conclusion is direct: “we have found no evidence to suggest benefit from biotin supplementation” in healthy individuals. That doesn’t mean biotin does nothing for everyone taking it (individual deficiency status usually isn’t tested), but it does mean the broad “biotin for better hair” claim runs well ahead of what controlled research has actually shown.
